Treatment of Nonunion of Femoral Neck Fracture Using Impacted Cancellous Allograft and Quadratus Femoris Pedicle Bone Graft

Abstract
Purpose: This study is attempted to evaluate the effects of impacted cancellous allograft and quadratus femoris pedicle bone graft in the management of nonunion of femoral neck fracture.

Materials and Methods: Between March 1998 and June 2002, 9 patients were treated with impacted cancellous allograft and quadratus femoris pedicle bone graft. All cases were nonunion from displaced transcervical fracture had been treated with closed reduction and multiple pinning. The average age of the patient was 38 years old (range, 29~51), and mean duration of follow-up was 85 months (range, 55~107). Clinical results were evaluated with Harris hip score and bone union was evaluated with 3 months interval radiographs.

Results: We obtained bone union in all cases. Mean duration of time to get union was 6.4 months (range, 5~8). Mean Harris hip score improved from 58 points (range, 43~72) preoperatively to 89 points (range, 83~94) at the last follow-up. 1 patient was rated fair and 8 patients were rated poor preoperatively, and 5 patients were rated excellent and 4 patients were rated good at the time of last follow up.

Conclusion: Impacted cancellous allograft and quadratus femoris pedicle bone graft is thought to be good treatment option of the nonunion of the femoral neck fracture for the patient with relatively good bone quality, because it can save the femoral heads.
